If you have looked at the App Store recently for a TRT tracker, you have seen at least a dozen results that all look similar. This is the ranking of seven that are actually worth installing in 2026, what each one is genuinely best at, and which one fits which use case. We make one of these apps (Calibrum). We rank it where it deserves to be ranked.

Limitations. Newer than TRT Plus (released May 2026), smaller installed base, fewer App Store reviews. Only one Free T calculation method (Vermeulen) — TRT Plus offers four. Less mature pharmacokinetic prediction than TRT Plus's Testimator. Audience framing is strictly clinical TRT, which is a feature for patients but a limitation if you want enhanced-athlete content. What it is. TRT Plus is the longest-running and most feature-rich TRT app on iOS. Strengths. Four Free T calculation methods. Testimator PK predictor. E2 modulator knowledge base with 23 research-backed entries citing Bhasin et al. (2018), Sniderman et al. (2021), and other peer-reviewed sources. Bulk lab upload (up to 15 images). Pathology partner integration. Large installed base since 2023.

Limitations. The breadth of features makes it less approachable for new TRT patients who just want a clean injection log and a coach. Best for. Users who want a completely free option with no paywall and no subscription. Users who prefer a web app accessible from any device over a native iOS app. Users whose primary goal is the shareable doctor report.

Strengths. Free with no premium tier. Web-based, works on any device. Solid blood-test tracking with reference ranges. Symptom correlation analysis. Shareable PDF reports.

OptiPin — Best for Peptide Users Alongside TRT

What it is. OptiPin is the most peptide-friendly app in the category. Tracks TRT alongside peptides (BPC-157, TB-500, ipamorelin, others) and GLP-1 medications, with a visual injection site map, dosage calculators, and inventory management.

Best for. Users running peptide protocols alongside TRT and wanting one app to handle both. Inventory-conscious users who want to track remaining vials and expiration dates. Users who want dosage calculators across multiple compounds.

Strengths. Multi-compound support (TRT, peptides, GLP-1). Strong dosage calculator suite. Inventory and expiration tracking. Visual injection site map across compound types.

TRT Pro — Best Pharmacokinetic Education Tool

What it is. TRT Pro (App Store ID 6462788647) is an educational tool with strong pharmacokinetic modeling — what your enanthate or cypionate is doing as levels fall between injections, plus a "try a new dose" simulator that shows what your levels would be if you change protocols. All methodology disclosed and backed by physician- or university-reviewed pharmacokinetic equations.

Best for. Users who want to understand the math of testosterone PK before changing protocols. Patients considering a switch from weekly to twice-weekly injection schedules who want to see the predicted level curve. Educational-focused users.

Strengths. Disclosed pharmacokinetic methodology. Solid level prediction simulator. Educational focus.

Why we ranked our own app first

Honest listicles that put the publisher's product at #1 without explanation are not honest listicles.
